Essentials of Case Management Class. 

This 5-week program is a comprehensive program for both newcomers and seasoned case managers on the history of case management and an understanding for how a highly effective case manager should function. Discussions will include how a breakdown of working with your patients, your interdisciplinary team, and how to effectively manage your day to identify resource utilization and progression of care needs. In addition, the course will discuss how to integrate social determinants of health (SDoH), readmission prevention, successful transitions for the discharged patient, and vital aspects to track and influence patient avoidable delays.

Program Objectives

  • Identify three ways Conditions of Participation affect daily case management practices.
  • Explain the four parts of Medicare and coverage by each entity that affects patient care, placement, and copays.
  • Design improved standard daily work that extends case management beyond the acute setting, providing wholistic management of a patient even after they are discharged from the inpatient setting, to keep them from being readmitted.

This program has been PACE certified by The Commission for Case Managers (CCM) for 5.0 CEUs. Weekly one-hour classes will begin in November.
