This class offers a strong foundation in hospitalized patient care and supports case management support staff in developing the skills needed to confidently succeed in their role. The target audience is any support role to the case management department. The class is a live virtual 1-hour class for 3 weeks.
IN-PERSON COURSES

This course covers all the essentials for working successfully in frontline case management. Topics include the history of case management, regulatory requirements, and how to be proactive in a CM role through progression of care and resource management.
Duration: 1 day
Additional Topics: acute care case management (including the continuation of care to the outpatient setting to improve patient care and outcomes), identifying appropriate patients for CM services, providing a comprehensive assessment, and how to put together a holistic transitional care plan and incorporating new SDoH requirements.
This course offers CE credit through PACE Commission of Case Management 6.25 hours and ASWB for Social Work CE credit of 6.25 general hours.
