Celebrating the Moments Behind the Scenes

By Tiffany Ferguson, LMSW, CMAC, ACM, FCM, ACPA-C

This week, I want to recognize and celebrate the profession of case management. ACMA and the Commission have named this year’s theme “Driving Outcomes,” while CMSA calls us to “Reconnect, Recognize, Reignite, and Rise.” As I reflect on a field I have called home since 2009, I want to honor those who have committed themselves to serving people during their most vulnerable healthcare journeys, helping them find a path through a system that can feel both convoluted and overwhelming.

According to the CMSA Standards of Practice (2022), case managers are defined as healthcare professionals who serve as patient advocates to support, guide, and coordinate care for patients, families, and caregivers as they navigate their health and wellness journeys.

Our work may not be glamorous. It may be marked by heavy caseloads, needs for which no resource or easy solution exists, and moments when we must deliver difficult news. We are rarely the ones standing front and center; that space is often reserved for our physician and nurse colleagues. Yet the connections we make, the burdens we help carry, and the lives we touch are no less meaningful.

Thank you to my fellow case managers: for every hand you held when a patient could not imagine how they would leave the hospital; for every middle-of-the-night call you answered when the medical team needed you to return and help untangle a difficult social situation; for every set of clothes you found after the trauma team had cut theirs away. Thank you for the housing you secured, the bus tickets you provided, and the countless acts of charity care you approved, knowing full well that a patient had no way to pay for medications, rent, COBRA coverage, transportation, or the long-term care they desperately needed.

Thank you for helping travelers find their way home; for asking the nurse to check once more when something did not seem right; for every phone call you made to offer medical education and for the calls when the most important thing you offered was the grace to listen.

This profession is not for everyone. But for those of us who keep showing up, sometimes all it takes is one thank-you, one smile, or a moment of laughter over coffee with colleagues. Sometimes it is the shared glance after a story so improbable that all we can say is, “You could not make this up.” These are the moments that remind us why the work matters.

Today, this week, and in all the unseen moments still to come, I celebrate us and I celebrate you. Happy Case Management Week.
